Avante TelAdvance, Inc. d/b/a Check 'n Go Online ('Check 'n Go Online' or 'Company') may be used in connection with publicizing and promoting the Company or one of its affiliated entities ('Companies'). I authorize the Companies, their agents, successors or assigns, permission to copy, exhibit, publish, distribute or record my name, likeness, image, voice, sound effects, interview, comments, questions testimonials, and/or performance in printed materials, multimedia presentations including film, tape, or photographs, on websites, or in any other distribution media (the ÒRecordingsÓ) for the purpose of publicizing the Companies programs or for any other lawful purpose. In addition, I authorize the Companies to edit such Recordings as they may desire, and incorporate such Recordings into all materials that are developed in connection with a marketing or public relations program. The Companies may use and authorize others to use the Recordings, any portions thereof, in all markets, manner and media including but not limited to printed reports, screenings, festivals, educational programs, websites, and broadcasts, without receiving my prior approval. I agree that I will make no monetary or other claim against the Companies for the use of the aforementioned.
Further, I waive any right to inspect or approve the finished product, including written copy, wherein my likeness or my testimonial appears. I hereby hold harmless and release the Companies from all claims, demands or causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate may have by reason of this authorization.
